Coaching Agreement

Description of Coaching: Coaching is a partnership (defined as an alliance) between the Coach and the Client in a thought-provoking and creative process that inspires the client to maximize personal and professional potential. It is designed to facilitate the creation or development of personal or professional goals and to develop and carry out a strategy or plan for achieving those goals.
The Nature of the Life Coaching Relationship
I understand that the purpose of my relationship with my life coach is to create, develop, and facilitate my personal and professional goals.
I understand that the purpose of life coaching is to develop and to implement a strategy, plan, or program that is designed to achieve those goals.
I understand that life coaching is not to be used in lieu of professional advice. I will seek professional guidance for legal, medical, financial, business, or other matters. I understand that all decisions in these areas are exclusively mine and I acknowledge that my decisions and my actions regarding them are my responsibility.
I am aware that I can choose to discontinue coaching at any time.
I understand that although life coaching is a process that may involve several areas of my life. Including but not limited to career, work, finances, health, and personal and professional relationships. Deciding how to manage these issues and implementing my choices is solely my responsibility.
I understand that in order to enhance the coaching process, I agree to communicate honestly, be open to feedback, and to create the time and energy to participate fully in the process.
Life Coaching and Psychotherapy
I understand that life coaching is not psychotherapy and neither treats mental disorders nor conducts mental health evaluations.
I certify that if I am currently in therapy or counseling, or otherwise under the care of a mental health professional, that I have consulted with this professional about my working with a life coach. I also certify that this mental health professional is aware of my decision to enter into a life coaching relationship.
I understand that life coaching is not a substitute for counseling, psychotherapy, psychoanalysis, mental health care or substance abuse treatment and will not be used in place of any form of psychotherapy.
The Nature of the Academic Coaching Relationship
I understand that the purpose of my relationship with my performance coach is to create, develop, and facilitate my personal and academic goals.
I understand that the purpose of academic coaching is to develop and to implement a strategy, plan, or program that is designed to achieve those goals.
I understand that academic coaching does not guarantee any outcome or result in client academic performance. I will seek professional guidance for legal, medical, financial, business, or other matters. I understand that all decisions in these areas are exclusively mine and I acknowledge that my decisions and my actions regarding them are my responsibility.
I am aware that I can choose to discontinue coaching at any time.
I understand that in order to enhance the coaching process, I agree to communicate honestly, be open to feedback, and to create the time and energy to participate fully in the process.
The Nature of the Presentation Coaching Relationship
I understand that the purpose of my relationship with my presentation coach is to create, develop, and facilitate my personal and professional performance goals.
I understand that the purpose of presentation coaching is to develop and to implement a strategy, plan, or program that is designed to achieve those goals.
I understand that academic coaching does not guarantee any outcome or result in client presentation performance I will seek professional guidance for legal, medical, financial, business, or other matters. I understand that all decisions in these areas are exclusively mine and I acknowledge that my decisions and my actions regarding them are my responsibility.
I am aware that I can choose to discontinue coaching at any time.
I understand that in order to enhance the coaching process, I agree to communicate honestly, be open to feedback and to create the time and energy to participate fully in the process.
Records & Confidentiality
I understand that information transmitted by me in this coaching relationship will be kept strictly confidential unless I give explicit written permission to release it to specifically designated persons. I understand that the only exception to this confidentiality will occur if the release of personal information is required by law.
Schedule and Fees
The time of the coaching sessions and/or location will be determined by Coach and Client prior to each session. The Coach will initiate all scheduled sessions and will begin promptly at the time of the scheduled appointment. In case of emergency, coach reserves the right to modify session time and date. Client will be informed and the session will be rescheduled.
Session fees are as follows:
$100 USD per 60-minute session
Payment is due via assigned service provider (PayPal or Stripe) before the start of each session.

Services
The parties agree to engage in the Coaching Process through in-person or video chat. Phone, text, and email communications are to facilitate the coordination of services rendered.
Cancellation Policy
Client agrees that it is the Client's responsibility to notify the Coach 24 hours in advance of the scheduled session. Coach reserves the right to bill Client for a missed meeting. Coach will attempt in good faith to reschedule meeting.
Termination
Either the Client or the Coach may terminate this Agreement at any time with written notice. Client agrees to compensate the Coach for all coaching services rendered through and including the effective date of termination of the coaching relationship.
Limited Liability
Except as expressly provided in this Agreement, the Coach makes no guarantees, representations, or warranties of any kind, expressed or implied, with respect to the coaching services agreed upon and rendered. In no event shall the Coach be liable to the Client for any indirect, consequential, or special damages. The Coach’s entire liability under this Agreement, and the Client’s exclusive remedy, shall be limited to the amount actually paid by the Client to the Coach under this Agreement for all coaching services rendered through and including the termination date.
Entire Agreement
This document reflects the entire agreement between the Coach and the Client, and reflects a complete understanding of the parties with respect to the subject matter. This Agreement supersedes all prior written and oral representations. The Agreement may not be amended or supplemented except in writing signed by both the Coach and the Client.
